2004 Chicken Surveillance Log

After loggin In select (click on)Chicken button located in upper right below Official Access on the right column of web page. Enter your user password (refer to user password guide to obtain access permission).
The Chicken data entry form will appear. (It will display in a new web page.)
Enter Information in the following fields.
Enter To Database- by pushing button entitled Submit this will enter information into the database and provide you will unique sample ID. Once selected a form will display with the sample Id information located in the last column entitled SampleIdNo. Use this number to mark the sample that will be sent to the lab. Use this form to obtain sample information and review for correction.
If correction is necessary then select back button on web browser to return to previous page. Note you will need the sample Id number to retrieve the information to edit. You may simply press the forward arrow in the web browser to obtain it. Press the clear form button and then Enter the SampleIdNo in the box at the bottom of the form with the edit button next to it. Press the edit button all the information will reappear in the form. Now change the infomation that needs to be corrected and press submitt. To verify that corrections have been made press the clear form button and then re-enter the SampleIdNo in the edit box and press edit button. The corrected information will apear on the form.
To continue entering data you must press the clear form button.
If no correction is need continue data entry as described below or exit all forms by selecting the X button located in the right corner of web page.
If additional samples
Select the Back Button on Web Browser for Data-Entry for a new sample [or repeated sample from the same locations]. All information from the previous sample will be maintained in the form change the Bird Serum No. and any other information that relates to the new sample. In order to obtain a new sample Id you must enter a unique Serum No. Otherwise you will get an error.
To obtain a list of all samples enter for the day to send to the lab first select the parish at the button of the form below the edit button and the date of data entry it can be one data or a range of days. Select print for a report of all samples that will be shipped to the lab for a single day. Print the form by selecting the print button on your browser.
