2004 Dead Bird Surveillance Log

 After loggin In  select dead bird button located in upper right below Official Access on the right column of web page. Enter your user password (refer to user password guide to obtain access permission).

The Dead Birds data entry form will appear .  (It will display in a new web page.)

Enter Information in the following fields.

 Select Bird Type, Raptor or other, if Other is selected enter species information on bird in box provided.  If new bird type occurs repeatedly contact steps@tulane.edu to have entered in Bird Type drop down box.

For Quantity Collected and Reported this refers to only samples not sent to the lab.  These provide a means to track additional dead birds in which samples can not be taken from or have not been sent to lab.  Collected refers to samples brought to Health units but not shipped to lab and reported refers to birds seen or found in the field but not brought into health units.  Every dead bird will need to be entered individually.

Enter To Database- by pushing button entitled Enter To Database this will enter information into the database and provide you will unique sample ID.  Once selected a form will display with the sample Id information located in the last column entitled SpecNoUse this number to mark the sample that will be sent to the lab and print the form by selecting the print button on your web browser to include this page with sample.  Exit all forms by selecting the X button located in the right corner of web page.

If additional samples

Select the Back Button on Web Browser for Data-Entry for a new sample [or repeated sample from the single location]

All information from the previous sample will be maintained in the form. Change any information that relates to the new sample for instance if it is a crow instead of crackle, and all other information will remains the same. 

Press Enter to Database to obtain new SpecNo for sample identification and new form to print for lab. If adding repeated collected or reported in field follow same method as above. They will be assigned a specno for internal purpose.

Report

Select the parish from the drop down list at the bottom of the screen, enter the to and from dates to create a report of samples entered over a certain time period, or to only print one form to send to lab for a large number of sample enteries.
