2004 Wild Bird Surveillance Log

After loggin In select Wild Bird button located in upper right below Health Official Access on the right column of web page. Enter your user password (refer to user password guide to obtain access permission).

The Wild Bird data entry form will appear.  (It will display in a new web page.) Enter Information in the following fields.

  1. Bird Type - select from drop down list, if bird not on list please fill in nam in other box
  2. Date Entered- automatically entered change if so needed.
  3. Bird Serum No – Enter the bird serum number this number should be unique for your organization.
  4. Band No. – enter address band number it is a required field.
  5. Sex – enter sex
  6. Age – enter age of bird in months 1-12 or 24 or 36
  7. Date Bled – enter date bird was bled
  8. Collection Site – Select collection site from drop down menu. If new site click new site location to enter information on new site

To enter information to Database- push button entitled Submit this will enter information into the database and provide you will unique sample ID. A form will display with the sample Id information located in the first column entitled SampleIdNoUse this number to mark the sample that will be sent to the lab.  Use this form to review sample information. 

If correction is necessary then select back button on web browser to return to previous page.  On the data entry form enter SampleIdNo Number next to edit.  Press edit information will appear on form. Make correction and then press submit.

To verify the data was corrected select press edit button will sampleIdNo entered.. Enter SampleIdNo  in box next to button press edit then press edit button.  To exit form press X located in upper right of web browser.

If additional samples (and no corrections made.)

Select the Back Button on Web Browser for to return to the Data-Entry form for a new sample record.  All information from the previous sample will be maintained in the form change the Bird Serum No. and any other information that relates to the new sample. In order to obtain a new sample Id you must enter a unique Serum No.

Reports

To obtain a list of all samples enter for the day to send to the lab.   First select the parish at the bottom of the form below the edit button and the date of data entry. It can be one date or a range of days.  Select print for a report of all samples that will be shipped to the lab for a single day.  Print the form by selecting the print button on your browser.
